There are 2 possible reasons the values you expect in the form do not show up:

  1. Duplicate record

    This issue is frequently encountered when the login feature is enabled. The form's capability to retrieve only one record at a time becomes problematic when multiple records share the same login value.

  2. Prefill

    The presence of prefill in the URL can lead to certain field values being replaced, affecting how they are displayed in the form.
