How to track which employee filled out the form?

Use a Portal with a users table linked to the submissions table to associate new submissions with the logged-in employee. A standalone Form login for viewing or updating an existing record does not, by itself, add employee attribution to unrelated new submissions. See the login guide for the two workflows. Returning users can reuse the stored login token while it remains valid.
